Carbon build up.
Sticky or stuck EGR valve.

Your most likely cause is a throttle position sensor, or a fuel problem, or restricted air flow.... that is a tough one.
I had the problem clean the egr valve, camshaft position sensor and the mass flow intake sensor use crc cleaner and brake cleaner for the egr valve. After all that i found out i had to adjust my throttle cable and adjust the idle it runs smooth now.

i have the same truck and same code , it seems that there is a problem with the egr vsv electrical valve connected to the egr and it's not the modulator valve the one that looks similar to the egr but plastic this one has two hoses to it and an electrical connector
po405---egr flow sensor low input-- this basicly means that the egr or connections to it need attention. remove and clean egr to get carbon build up out of it. also make sure it is clean at motor side where it mounts to engine, if no carbon replace egr valve.
The OBD 2 code, P0401 means "low flow". The most common cause of this code with any Ford vehicle is the DPFE sensor, NOT the E.G.R. Other causes could be blocked passages for the EGR, or leaking or restricted hoses that go to the DPFE. == ==
The most likely cause of low-power, when under load, in your Cummins N14 diesel engine is a lack of fuel. The problem can also be a lack of air flow.
