They should be alert to signs of abuse including neglect, physical, psychological, and sexual. If they have not been trained to recognize those signs, they should seek out training. There is plenty of training available. In addition, pediatric caregivers should be aware of the cyber threats to children including exposure to adult materials on-line including violence, hate, and sex materials. They can discuss children's on-line habits with them while providing care and counsel them about protecting themselves. Discussions about dealing with depression, on-line bullying, attempts at on-line seductions, and the dangers of sexting can be discussed. If the caregiver discovers that the children are at risk, they should discuss it with the parents, and if necessary, report it to the authorities. The pediatric caregiver can also be a resource for education for the parents - alerting them to risks, problems, and ways to protect their children.
Being gay is not a reason to "protect children". Unless there is another reason you need to protect your children from your spouse, the answer is that you need more help than your spouse does.
