With regard to evolution, it is generally the interpretation of Genesis as a literal, scientific document. People with a dogmatic view of The Bible are often very hostile toward science, as they believe that anything that contradicts their understanding of the Bible must be false, even if it has been proven scientifically. Having said that, I have nothing against fundamentalists.

There were several factors that caused disunity during the Middle Ages. These include political fragmentation, religious conflicts (such as the Great Schism), feudal system disputes, and the rise of competing power centers like city-states and monarchies. Religious thought and ideas changed somewhat - Lollards, Hussites, Martin Luther and Calvin all rejected, in one form or another, Christian Doctrine as presented by the Catholic Church. However, this is more to do with religious doctrine than philosophy as such.

Yes. The Church of Jesus Christ of Latter-day Saints (the "Mormon" church) believes in apostasy, or the concept of 'falling away' from true doctrine. They believe that shortly after the death of the Apostles, the Christian world fell into a state of apostasy. This caused the need for the true Christian Church to be restored to the earth - which they believe was accomplished in the founding of their Church in 1830 through Joseph Smith, Jr. Mormons also believe in the apostasy of individuals. Those who leave the Church over disagreements with doctrine are considered apostates.
