Well...the Black Death didn't quite die out. It still exists. The Great Plague itself went through several cycles some years apart. There were three major pandemics and many smaller ones. To this day no one is quite sure what the specific disease was. Changes in living conditions, changes to the some immunity in the population, decreases in the rat and rodent population and just plain old time caused it to become scarce.
