The first thing that comes to mind is that the permanent first molar is beginning to erupt. This is normal for his/her age. It may be tender for a while until the cusps break through the gum tissue.
Another possibility is that the last tooth may be infected due to decay and the infection is spreading to the gum immediately behind the tooth. Any qualified dentist should be able to make the proper diagnosis in about two minutes.
