I believe that this is caused as a result of rough intercourse or simply the lenght of the penis. I had this problem with some ex-girlfriends and not because I am anything special. My member is of average width but a little longer and it seems that because of this I kept hitting sensitive parts. This used to cause alarm at first but now when it happens I simply explain what I consider to be the reason even though it is a little embarrassing

Women are born with a very thin layer of skin called the hymen covering the entry to the vagina. Sometime it cover the opening completely and sometimes it just forms a ring around the opening. The first time the vagina is penetrated with anything large enough to stretch the opening, like the man's penis, this thin layer of skin tears and can bleed. It doen't always tear and it doesn't always bleed, but if you see a small amount of blood at first intercourse this is probably what has happened - the tearing of the hymen.

This would be a result of the hymen being broken. The hymen is a thing layer of skin just at the vaginal wall. It can also be broken using tampons on occasion.
