Example: a man who is 200 lbs overweight, has a stressful job, eats crap all the time and smokes three packs of cigarettes per day gets in a wicked argument with his wife and drops dead. The immediate cause turned out to be acute stress but that's not the whole story. The complete picture includes underlying causes i.e. obesity, smoking, stress (and everything that comes along with these: atherosclerosis, high blood pressure, coronary artery disease); it is all this that really set the guy up to drop dead from an argument.
