Mentally ill patients don't care. They are doing all they can just to get out of bed and function - and some of them can't manage that. They just don't have the energy to care about taking care of themselves. This is why you see people with severe mental illness wandering around unkempt, unwashed and unshaven, and with decayed teeth.
