I am not an expert, but here's what little I know. Someone else can add more I'm sure.

First - it wasn't just the Government who helped. As at all times, many private charities and individuals helped bear the weight of caring for their forebears. Indeed, without such heartfelt concern on the part of direct caretakers, no program will ever be enough.

For centuries prior, elderly who could no longer help themselves and had no family to care for them would live in the same institutions as other poor and infirm. Nursing homes as such did not begin appearing until the 1800s.

As of the beginning of the 1930s, many elderly still lived in almshouses. The big thing that helped to change this at that time was the establishment of social security. I don't have enough of a grasp of it to explain it, but more info can be found here if you are willing to do some reading.
