It pulls the cool air from outside of the engine compartment through the radiator to cool the hot anti freeze for circulation though the engine block.
The relay turns the fan on/off with a command from the thermal switch.
Swap the fan relay with another like relay.

Before you fix the Fan Relay, look for a "RAD FAN" fuse (30 amp - orange). Normally, the computer diagnostic shows the problem is the Fan Relay, but, in my case was the Radiator Fan Fuse ($5). I waste $60 paying for the Fan Relay :-[

The cooling fan relay switch can be located on the back of the cooling fan motor. The relay switch should be labeled as such.
If you are talking about the radiator cooling fan it could be the fan fuse or the radiator fan relay needs to be replaced... the fan relay is VERY common for these vans. I have a 98 T&C and just had to replace the fan relay. The Rad Fan relay cost $40.00
