If the aunt is your aunt by blood (the sister of one of your parents) then her mother's father's wife would be your great grandmother. If your aunt is the wife of your blood related uncle, then I am not sure if there is a kinship designation other than extended family. If this is a person who is alive and part of a family gathering or special event, I would call her great grandmother out of respect, even if there is no blood connection.
