you can get HM rock climb from oak and then he says mt. silver is open and you can vs. trainer red then beat him, go back to oak and he will give you 1 kanto starter
To elaborate more:
You can now do several things, most pertain to legendary Pokemon.
1) Catch mewtwo. He is in the same cave just north of Cerulean city. Bring a Dark type Pokemon as Mewtwo's only damaging attack is Psycho cut.
2) Catch the legendary birds. Zapdos is next to the power plant, Moltres is in Mt. Silver(see #3 below), Articuno is in the Seafoam islands, on the first island you come to Surfing from Fuschia.
3) Go to Mt. Silver. Go back to the Pokemon League. You can now take the left hallway at the crosspath. This takes you to Mt. Silver. Both the route leading to it, and Mt. silver itself is full of good Pokemon (generally levels 40-60).
4) Fight red. Red, the main character from Pokemon Red/Green/Blue/Yellow/FireRed/LeafGreen is waiting at the top of Mt. Silver to face you. His Pokemon all top level 80, biggest is his lvl 88 pikachu.
4a) After you defeat Red you can go to Prof. Oak in pallet town and he will give you one of the original starters (charmander, bulbasaur or squirtle) as well as the Blue orb (Gold) or Red Orb (Silver).
4b) Catch Groudon/Kyogre. Take the Orb that Oak gives you to the cave to the far southwest of Johto, below the Safari Zone. Inside you will find Groudon (or Kyogre in Gold).
5) Get vintage audio. Go to the condos in Vermillion city where the "Gamefreak" office is and talk to the sound designer. If you have all 16 badges he will make it so audio from the original games plays at certain points on the map.
6) Pokeathlon. Nothing really unlocks but if you go downstairs in the pokeathlon dome there is a lot of stuff to unlock if you haven't already.
7) Catch Suicune. Again not specifically unlocked, but if you haven't made the effort yet you should have seen him at most of the spots you need to. The last 3 you may have missed in order are:
Vermillion City - Walk down the dock to see him.
Rte. 14 - In the water on the east side of the path.
Rte 25 - Here he will sit and you can catch him.
Get shiny leaf to get a 2 star of trainer card
Defeat Pokeathlon to get a 3 star of trainer card
Have 100 strike at battle tower to get a 4 star of trainer cardHave captured not Sean all 493 Pokemon to get a 5 star of trainer card

Things you can do in Pokémon HeartGold after beating Blue include training your Pokémon to Level 10, receiving a Lunar Wing from a guy outside in Pewter City in order to go to the go to Whirl Islands which is in the water route in the Johto region between Olivine City and Cianwood City, you will required Pokémon that know the HMs of Whirlpool and HM Surf. In the Whirl Islands, due to having the Lunar Wing in your possession, you will be able to encounter a wild Lugia so that you will have a chance to catch it. Lugia will be at Level 70.
You can also catch Legendary Pokémon such as Articuno, Zapdos, Moltres and Mewtwo. Articuno is in the Seafoam Islands, Zapdos is near the Kanto Power Plant, Moltres is in Mt. Silver and Mewtwo is in Cerulean Cave.
After getting all 16 Badges, you can catch Mewtwo in the Cerulean Cave, catch Articuno in the Seafoam Island, catch Zapdos near the Power Plant, catch Moltres in Mt. Silver, receive a Kanto Starter Pokémon from Professor Oak in his Lab, receive the Red Orb from Mr. Pokémon and catch Groudon in the Embedded Tower which is west of Cianwood City, get the GB Sounds in the Celadon Condo on its Game Freak floor which is the third floor, catch Ho-Oh in the Bell Tower once a Pewter City character has given you the Rainbow Wing, catch Latios however it will be roaming around in the Kanto region after you leave the Pokémon Fan Club and you can also receive 1 of the Hoenn Starter Pokémon from Steven Stone, he will be found in Silph Co. however you need to beat Red and also activate the Latios roaming event.

Go to Prof. Oak and he will say something about going to Mt. Silver. Go there and climb to the top and defeat Trainer Red (he has levels 80-88).
