There may be a couple reasons. It could be not pushed in all the way (sometimes it needs an extra little jab) or it could be a scratched disk. Or it could be the system itself. Usually I find its the first one though.
Or it depends what disk your trying to play in your ps3. If its a bootleg disk they now have whats called cinavia protection. So the ps3 can only play some bootlegs and not all.
