It means to avoid every kind of evil.
"Appearance" can mean:
1. The act of appearing.
2. The outward aspect of something.
3. A pretense
"Abstain from the appearance of evil" does not necessarily have to mean "Abstain from what looks evil". Many tend to misquote the verse to mean that you should avoid everything that even looks evil. However, that interpretation would be contradictory to The Bible. Jesus himself did many things that looked evil, such as eating with prostitutes and performing miracles on the Sabbath. It surely looked evil, but was it? No, Jesus never sinned.
