You have a bad o2 sensor (low voltage)
it depends on the model of engine, my 97 rodeo with the 4ze1 motor has the map sensor for air intake but doesnt have an O2 sensor for the exhaust.
the o2 sensors are located on the exhaust pipe one by trans and one by the muffler there is 2 of them
its on the end of the wires just above the air filter housing unit
the bank 2 is on the driver side i got this info from an Isuzu mechanic

i have just gone through this same situation, in my case it follows with i think 1157 or 1137 regardless they are o2 sensor codes. funny enough this , in my case, led to the MAP sensor found center rear of intake manifold. worked for me 100 CAD new pop it out and replace it should be good as new! Bank 1 is the side that cylinder 1 is on, sensor 1 is downstream and sensor 2 is upstream

Mine has 4 O2 sensors, one before, and one after the CAT on each bank. (just a FYI, I was getting an O2 lean error caused by the short flex pipe just after the Y leaking)
On an Isuzu Axiom, the O2 sensor is typically located in the exhaust system. There are usually two O2 sensors: one positioned before the catalytic converter (upstream) and another after the catalytic converter (downstream). I'm not a mechanic, but I'm pretty sure you have one O2 sensor just before the catalytic converter and one after. The first checks the function of, and regulates, your fuel control unit, and the second checks the efficiency of your catalytic converter. My #1 O2 sensor is screwed into the exhaust pipe about a foot or two before the converter and kind of looks like a spark plug. On a 2003 Isuzu the O2 sensor is on the driver side of the engine compartment about a foot down from where the exhaust pipe comes out of the header. Just follow the exhaust pipe out of the engine and you will see it. It looks like a spark plug sticking out. It's pretty easy to disconnect the wires, unscrew it and replace it.
