When writing in a patient's legal medical record, you are recording events and issues as they occur. Using medical terms denotes that you are competent, and educated. If poor spelling and grammar are present, it immediately poses to question to the reader: "Is this the person that I want to be treating me/my patient and documenting my illness and issues?". Another issue is that if a medical term is misspelled, it could actually mean something else! For example, if a patient is to have a medication "QOD", but the spelling is incorrect and written as "QD", then you are basically saying that the patient should have the medication EVERY DAY (QD) when it was meant to be taken EVERY OTHER DAY (QOD).
