It refers to a body (human or animal) that is unhealthily gaunt, undernourished, or emaciated; one with no fat, but equally, no muscle. It has a negative connotation beyond 'skinny,' and shouldn't be confused with 'thin' or 'lean'. Ex: My heart went out to the orphan girl, she was all skin and bones.
