Another, objective viewpoint is needed.

Very generally, it might represent the dreamer's anxiety about "tests", whether actual medical tests or, more symbolically, academic or professional tests, including annual evaluations or interviews. A different possibility might be some physical irritation, such as an insect bite "poking" the sleeper. Yet another possibility is that the dream could express the dreamer's feeling about some irritating person who keeps "poking" and "needling" the dreamer in real life.
