A rational non-religious person would say you are imagining a punishing judge when it is simply your conscience bothering you; simply admit you did a bad thing that now you regret and move on.

Finally, it could be said that it is about a serious concern about someone else's (likely close to you) health that is not being taken care of properly. Or perhaps it is referring to a mundane or a serious problem (such as a car repair or maintenance that you know you need) that you are trying to push out of your mind.
