Usually from my experience it happens when I start to do really well in a video game. For example, it happens to me alot when i play COD MW3 and Im doing really well at sniping it either says lost connection to host or server error or timed out. I assume it has something to do with the host connection or in some very rare cases a "DDOS" which is something that people can do to kick you offline. Yes, that means they are a hacker.
it means the server has gone down such as minecraft server has crashed or the server owner has lost Internet connection

There might be a problem with your broadband/Internet or maybe your connection to the network from your xbox! But if it says your account has been recovered onto a different console then you may want to call Microsoft because there maybe a possibility that you have been hacked!

if it says its timed out you have the wrong password. you can figure out the password or remove using your computer

if you have an internet connection through land line you can get a Lan cable and hook it up from the back of the xbox. i think it says ethernet but not sure. them get xbox live membership and your ready or you can use xbox live free
