It's actually meant pretty well literally, by a doctor or dentist to try to reassure the patient who knows or expects the impending injection or other uncomfortable treatment will hurt... a bit.
I've found doctors and nurses will warn me it will hurt - a bit. They know patients don't believe the cliche so prepare them for the worst; with the consequent pleasant surprise that actually, it was not too painful at all.
The sentence might be used idiomatically, or metaphorically, though I've not encountered this.
