Polonius plans to have Hamlet meet with Ophelia while he and Claudius hide nearby so they can watch how he acts towards her. This way they will be able to see if, given the opportunity, he will show affection towards her or just continue on in his madness.

Polonius and the King decide to test Hamlet by arranging a time for Hamlet and Ophelia to meet and chat. Polonius and the King would then hide and listen in. Hamlet, however, is able to tell that he was being listened to so he fakes his madness as he had done previously.

What was Ophelia's full name?

In the play she is only referred to as Ophelia. One might look at the naming customs in Scandinavia, but the characters in the play do not as a rule have Scandinavian names, except Hamlet himself and his mother (based on the names Amleth and Geruth in the original by Saxo). Claudius, Laertes, Polonius--these are classical names, not Germanic.
