This refers to when a person prays for someone who is need by laying their hands on them during their prayer. This is viewed by some as simply a way to associate with the person in need. Others see this as a transferrence of energy. An example of this was when the lady with the issue of blood touched the hem of Jesus's garment and was healed and Jeses said he felt "it" leave his body. Personally, I think it is an important part of prayer if you are interceding with someone on a 1-to-1 confrontation. Scientists have shown that it actually does effect people who are sick in the hospital when someone touches them while praying. The explanation may be complex and it could be pyschological or spiritual but if it works then don't deny it.
It represents the healing touch of Jesus Christ and it is a way to show comfort and that you are praying for them.
It is not a symbol. It is something that is done for example to pray for someone to receive the Baptism of the Holy Spirit.

the forehead, hand and heart
The repetition of the Baptismal vows, and the laying of the Bishop's hands on the candidate, so that he/she will receive the Holy Spirit.
.Catholic AnswerIf confirmation is separated from Baptism, it begins with the renewal of baptismal promises and the profession of faith. Then the bishop extends his hands over them and invokes the outpouring of the Spirit. The essential rite follows: in the Latin rite, confirmation is conferred through the anointing with chrism on the forehead, laying on of the hand, and the words: 'Accipe signaculum doni Spiritus Sancti" [Be sealed with the Gift of the Holy Spirit.]." (Paul VI, apostoloic constitution, Divinae consortium naturae, 663.) The sign of peace concludes the confirmation.
