If you started a 'New Game +' then your inventory slots and weapon slots are reset to how they were originally, the first time through. There's no way to 'fix' it other than continue playing to unlock them the same way you did the first time.

Borderlands checks weapons for legitimacy, that doesn't mean you can't still hack them, it just means you can only hack in a weapon that could technically be found through normal play. If you hacked the weapon to be a Lv68 gun usable by a Lv1 player, the game will detect this and the gun won't be in your backpack. Likewise, a Hyperion rifle with a Dahl grip is impossible to find in-game so will also not show. But you could edit a weapon you already hold, giving it optimal parts and increasing the quality to be the same as your own level, to give you the 'perfect' version of that weapon for your level.
