The W 4 comes from your employer so you can file income taxes.
You will need to submit a I-9, a state and local W4, and a Federal W4.

No, not really. If you change the status on your W4 from Single to Married, less will be withheld from your paychecks for federal income taxes. There is no requirement that you make that change.
IRS Form W4.
A person can pick up a 2012 w4 form at a couple of different location depending on where the person lives, such as the local library or the city hall as well.
There are two reasons you may have no federal taxes taken is if you claim exempt on your W4 or if you claim a higher amount of dependents.
If you go to your local government office there are employees there that will be more than happy to show you how to fill out a W4 form. When you pick up your W4 form there is also an instruction manual that will walk you through the proper steps for filling out the form.
It varies by state. Some states do not have a state tax withholding form - and in that situation an employee can submit a Federal W-4, designating that he/she is only changing their state tax withholding only. In addition, there are other states (i.e. Washington, Tennessee and Texas) that do not have state tax withholding at all.
