We really need context to be able to answer questions like this. There is no specific form that all corporations must "file" (with whom?) to terminate any employee.

Most states have a form that you need to file to have a person removed from a corporation. There may be a filing fee associated with this.
To remove this, a person needs to go into file and programs, and click remove program. A person can also clear the cookies and cache to remove part of it from the computer.
