When the events of the Nine-tailed happened 16 years ago, it was Madara Uchiha who stole the Nine-tails from her after she gave birth to Naruto (a female host is at her most weakest state when giving birth so Madara waited till she gave birth to steal the demon fox). After Minato (Naruto's father) fought Madara off, he sealed the fox into her newborn son. Whereas, he sealed a little of his chakras and the rest of Kushina's chakras into the seal to protect their son which in turn, took both of their lives.
