Leap year is just a way for man to keep up on the math, it has nothing to do with the sun or the Earth. A year is classed as how long the earth takes to travel round the sun. This actually takes 365.25 days, but for easiness sake we say it lasts 365 exactly. Only this left a quarter of a day each year unaccounted for and this would soon add up and mess up the whole system of measuring years, seasons and such. To this end it was decided that every four years, on extra day would be added to calendar year, as every year a quarter of a day was lost, so four quarters make a whole day. The extra day in the leap year has no effect on the earth or the sun, it is simply a way of keeping the calendar year the correct length.

Nothing happens to the Earth. Our calendar gets a day added on Feb 29th every four years to compensate for a year being 365.2322 days. Interestingly enough, since the year is not quite a day longer every four years, which would be 365.25 days, three days out of every four hundred years are removed. The days that are removed are in the leap years that fall on a century year. If the century year cannot be divided evenly by 400, then that year does not receive a leap day on Feb 29th. Nobody noticed this in the year 2000, because it can be divided evenly by 400. But the century years of 2100, 2200 and 2300 will not have a leap day added on Feb 29th.

Nothing special happens to the Earth on leap day. It is purely for the purpose of keeping our calendars in line with the time the Earth takes to orbit the sun. Our calendar year is 365 days, but the Earth takes 365.25 days to orbit the Sun, so every 4 years we take the .25 from each year and add them together to give one extra day.

How does the earth move when its leap year?

The Earth moves the same in a leap year as it does in any other year. We have leap years because Earth takes about 365.25 days to go around the sun, not 365 days, so a leap year makes up for that in order to keep our calendars correct.

Does a leap year occur because the tilt of the earth's axis changes?

No, a leap year does not occur because the tilt of the Earth's axis changes. A leap year happens to keep our calendar in alignment with the Earth's revolutions around the sun. It consists of adding an extra day, February 29th, to the calendar every four years. The tilt of the Earth's axis remains relatively constant.
