The outside unit will not defrost and a thick layer of frost will build up, rendering it innefective.
Assuming you are talking about a heatpump... Defective defrost timer board or thermostat.
do you have a heatpump or a/c? if it is a heat pump you can,that is what happens when the system goes in to the defrost mode. if its freezing you could be low on charge or or bad defroster board or even worst a bad reverseing valve.

The defrost timer, thermostat or logic board, whichever you have is probably bad. To quickly remove the ice build up on the outdoor unit switch it to run the ac for a few minutes or until the ice clears.

If it's stuck open, no heat. If it's stuck closed, over heat.

Yes both indoor and outdoor fans will operate, the only time 1 will stop in heat mode is in defrost the outdoor one stops briefly to defrost better,
