Smegma is a natural substance and does not by itself contain or cause disease. It is an organic substance and may become a breeding ground for germs that could cause problems. With the build-up of smegma there would be an obvious and unpleasant odor, and hygiene is absolutely essential. You don't have to scrub the area with harsh and drying soaps all the time, but daily gentle cleaning with water and mild soap would be enough.
