Shock
Vital signs are an indication of the patient's condition and can help diagnose problems. A pulse that is strong is an indication of a stable patient, for example. One that is weak and thready may indicate the patient is in shock or has other injuries. So the vital signs give the workers an indication of what needs to be done in order to stabilize and transport the patient.
The US spelling is spaghetti (the typical thready pasta, from the Italian).
A thready pulse is a weak pulse; so weak that it is very hard to find. This could be due to heart disease or injury. It could also be due to naturally fine veins and arteries.

I'm assuming that you heard this term from a practitioner of Chinese Medicine. If it's referring to something else, disregard this answer. In Chinese Medicine a thready pulse is one way to translate the Chinese term Xi Mai, which describes a pulse that is thin and most often weak in quality. It feels thin in diameter, like a silken thread (indicating a lesser volume of fluid in the artery itself), and usually is soft, and forceless. It indicates a physiological deficiency of some sort, usually of the qi or blood. Hope this was helpful. It is not only used in Chinese Medicine. It is commonly used in Canada to describe a weak pulse with varying degrees of strength. Thready pulse is also taught to be a key sign in Shock ( hypovolemic, anaphylactic, septic, cardiogenic and neurogenic shock )
As a rule of thumb, If you can palpate a brachial pulse the systolic pressure is at least 60 mmhg. Character of the pulse whether bounding or thready will give additional clues to the approximate blood pressure. If you can palpate a radial pulse the BP is at least 80 mmHg.
The most common error is when the professional looks at movement on the needle of the cuff as opposed to listening to the actual pulse in the artery. The needle inside the dial will often times "tick" at each beat, but it may not hear the first or last which is what makes up the blood pressure. Also, the cuff can be misplaced, without the tubing placed directly over the artery. Other errors-movement can cause incorrect reading, having had legs crossed can cause an altered reading, thready or low blood pressures may be hard to read and sometimes unreadable with digital cuffs.
