The form in the medical record that contains subjective info about the patient's past illness is called a SOAP note. SOAP is the acronym for subjective, objective, assessment, and plan.
There are no statistics kept on that. The info is considered privalage health info so not public record.
schools use databases to keep record of students which may include: first-last-middle name, age, grade, medical info, home address, parent contact info, nationality. etc.
