It is the top dome of the pressure vessel which is removed to enable fuel assemblies to be changed. It is held on by a ring of very heavy duty bolts. In the PWR it holds the control rod mechanisms, so before it is lifted off the control rods have to be detached so they stay in the core during fuelling. In the BWR the rods come up from underneath the vessel.
