Tsunade continues to be the fifth (Godaime) Hokage of the Hidden Leaf in Shippuden. Most notably, she defends the village during Pain's attack and gives medical assistance (via Katsuyuu) to every member of the village. She then falls into a coma because of this, and Danzo acts as Hokage (but never officially becomes one) while she recovers. However, certain things happen (to not spoil it too much) and Tsunade comes out of the coma to resume her position as Hokage. She is also a critical factor in coordinating the medical corps in the 4th Joint Shinobi war. This is her role up to the most recent manga chapter, 516.
