It's a prank coin made as a novelty item. Please use this site's Search feature to look for the word DOUBLE. You'll find hundreds of similar questions.
A 1993/1995 "two-headed" nickel is not a genuine coin issued by the U.S. Mint. These are novelty items created by altering genuine coins. The "P" mint mark would suggest it was minted in Philadelphia, but again, this combination is not authentic and holds no numismatic value.
2 headed or 2 tailed coins come from magic shops and are made by altering 2 normal coins. They regularly sell as novelty items for a couple dollars. While a couple of genuine 2-tailed quarters have been found, there has never been a genuine 2-headed nickel ever authenticated. See the related Web Links for more info...
