They need some form of help, likecritique and feedbackan editor to remove the fluff out of their storiesreaders who share the author's excitementAnyone who helped the author is their quest to become published or finish their book should be included on the dedication page.Loved ones, family members are friends are usually included in a dedication page as well.
It depends on a the book. There might be an introduction or preface. Other books have the dedication page where the page usually says "To ___," or "For___," or simply "dedicated to ___."

The title page is at the beginning of a book or document and shows the name of the work, the Author's name and sometimes a dedication. The Copyright page is usually at the end of the book and shows references to resources and has a copyright paragraph. The copyright paragraph states that no one can copy this article or work without the express permission of the author.

"Darkness sings"; The Book Thief, page 91. "Cold fire"; The Book Thief, page 240. "Bitter sweet"; The Book Thief, page 305. "Friendly enemy"; The Book Thief, page 389. "Silent scream"; The Book Thief, page 428.
It completely depends on what edition you have. Usually books are printed in New York, but there is an easy way to check with any book. Flip open the book to the page after the title, but before the dedication/first chapter. Next, look on the left hand page and you will see all the publishing information. Unfortunately, sometimes the publishing information is not very specific, and only says "United States".
