I think you are referring to "bag of WATERS", which encases the baby in it's liquid environment before it is born. When performing a Caesarean, the surgeon will break the bag with the tip of the scalpel prior to lifting the newborn out of the womb.
You don't always. I am a midwife and I thought my waters had gone but they had not. If the baby's head is well into the pelvis they may not go at all until the baby is born. However if you have an uncontrollable dampness (smells different to urine) your waters have probably gone.
