A hotfix is a patch that addresses a specific issue. This is in contrast to a service pack or rollup, which bundles several fixes into a single package.
in my humble opinion there are no difference for personal user except the huge file space, any way SP3 made some hotfix and some security update you can choose what you need from MS windows update center ( I choose Hotfix only )

Hotfix files can be removed but not recommended in case something happens in a new update and have to revert to an older update. Many of the Hotfix files/programs can be found in the add/remove programs list. From there you should be able to delete them. The files also like to hang out on other places within windows so you need need to do a file search to find them all.
You have been misinformed. You can have grapefruit or its juice when on the patch. It doesn't make a clinical difference in how the patch works.
