This is an ultrasound they do to see if your bladder is retaining too much water after you urinate.

(In my case anyway) they ask you to drink 500 mL of water, finishing an hour before your exam. You're asked not to urinate until your appointment. First they'll ask you to unbuckle your pants and push them down a little bit (just a little bit, not down past your genitals), they tuck a towel in, smear your lower belly with ultrasound jelly (suprisingly warm!) and they scan your bladder before urination. This is really the only potentially uncomfortable part as, if you really have to go by now, the scanner is being pressed down on your bladder. However this portion of the scan is less than five minutes long, so not to worry.

You're then asked to urinate (in a regular washroom, by yourself). You return to the scan room, and the same procedure happens again with them scanning your bladder area, only this time you will also be asked to turn on both sides as well so they can also get shots of your kidneys. This portion of the procedure takes a little bit longer, but again it's over quite quickly.

Actual scanning time altogether is probably fifteen minutes, certainly no more than twenty. There's no pain from the procedure itself (except perhaps if you have internal soreness, as in your kidneys, that might be aggravated by them pressing down with the probe?) and the embarassment factor is very low as well.
