A xanthoma remains intact, solid/firm, almost hard when removed - a whitehead does not - having the consistancy of pus when opened.
The symptoms are that you get a skin condition. A so called xanthoma looks like a sore or bump under the skin. Commonly it's flat, fairly soft to the touch and yellowish in color. Edges are sharp and distinct.
