This med is an NSAID. If administered PO, this med is absorbed in the gut and very quickly progresses through plasma, concetrating in synovial fluids. In fact, chronic application showed nearly double the plasma level of the drug in the synovial fluid (of the knee mostly). This med is very protein-bound. It metabolizes mostly in the kidneys into the main metabolite, 4`-hydroxyaceclofenac, and is excreted 80% through urine and 20% through feces.

This difference in the sequence of amino acids that stimulates the production of the antibody. This antibody in turn attack the similar looking body protein. Thus giving rise to so called autoimmune disease. Your knowledge of Microbiology is very elementary. It takes some knowledge to know that you are ignorant. So coming to the point. You know one disease called as rheumatic fever and Arthritis. It is caused by beta hemolytic streptococci. Here you get the bacterial infection that is fallowed by immunological response. Unless you treat the infection, the disease can not be cured. Two moths back a patient of rheumatoid arthritis came to me. He is a male patient, age about 24 years. He has been suffering from rheumatoid arthritis from last four years. There has been involvement of the small joints. One Rheumatologist did the blood test on 05/19/2014. C reactive protein was positive. (194 mg/liter.) R A factor positive. (24 I U/ml.) With ASO titre negative. Patient came to me on 04/11/2016. His ESR was 50 mm per hour of Wintrobe at that time. He was given tablet Azithromycin one gram stat fallowed by 500 mg BID for two weeks. Tablet Satronidazole 300 mg BID for two weeks. Tablet Albendazole 400 mg twice a day for two weeks. (To be chewed before swallowing.) Tablet serratiopeptidase 10 mg twice a day for two weeks. Tablet Fluconazole 150 mg on alternate day for two weeks. He was provided with ten tablets of Aceclofenac 100 mg for pain relief. Then patient got relief and disappeared. He came back with relapse of symptoms about some ten days later. ESR was found to be 45 mm by Wintrobe method. He was given the repeat course of the same drugs for three weeks. Tablet Serratiopeptidase, Albendazole and Fluconazole was continued for another one week. He was given analgesics. But he used only two tablets of Aceclofenac 100 mg. He was on no drugs for about ten days. On June/11/2016 (today) his ESR is found to be only 15 mm by Wintrobe method. The symptoms are very less. He is getting some morning stiffness of small joints. I am not sure about the cure of infection. To prevent the relapse of the infection, I gave him tablet Azithromycin 500 mg one BID today. To be fallowed by one tablet daily for 15 days. Tablet Satrozidazole 300 mg BID today to be fallowed by one tablet daily for next 15 days. Tablet Serratiopeptidase 10 mg BID for 15 days. Patient is asked to come after 15 days. I am supposed to continue the treatment for another one month and half from today on wards. The treatment is continued in anticipation that some unknown chronic infections may take longer period to get treated. The exact drug, dose and duration of treatment can be done at proper center by the expert in this field by permutation and combination method. Here I have to stress the important role of tablet Serratiopeptidase. I did not mention that I have given multivitamin preparation and a shot of vitamin D 3 of 6 lac units also. *20 days back the article was written. Patient was given further course of Tablet Azithromycin 1000 mg stat, fallowed by one tablet once a day for 15 days. Tablet Satronidazole 300 mg once a day with Tablet Serratiopeptidase 10 mg twice a day for 20 days. This was done as a precautionary measure to cure the unknown infection. The patient is free from pain and not taking any analgesic drugs. He does not need the same. * Patient came for fallow up after 40 days. No joint pains. No analgesics. ESR today was 7 mm/ hour by Wintrobe method. Rheumatoid factor is negative. Probably got cured till that unknown infection returns back. * Patient came for fallow up after about one and half months. Rheumatoid factor test is negative. ESR 8 mm / hour by Wintrobe method. Patient has got little morning stiffness in both wrist joints. He actively work in the farm for whole day. Not taking any analgesics. * Patient came after about one and half months. Rheumatoid factor test is non-reactive. ESR is 9 mm per hour by Wintrobe method. Patient have some joint pain after the cold whether began. He is given mild analgesics, Injection Vitamin D 6 Lac units and calcium salt preparation.
