When you are in ketosis. This can result from Diabetes. OR if you are on a high protein diet, like the Atkins diet. It is a sign you are burning your fat.

However, the Atkins diet has been shown to be inadvisable and does not even really induce ketosis. In fact, any weight loss is more-so the simple fact that protein is more filling than carbs, sugars, and fats. Not healthy either.
