Any 4 points in the Cartesian plane determine a unique equation that is of degree at most three (i.e., a "cubic" equation). It is, of course, possible that the 4 points actually lie on a degree two ("quadratic"), a degree one ("linear"), or a degree zero ("constant") equation. However, if the 4 points do not lie on a constant, linear, or quadratic curve, then they will like on a unique cubic curve. In general, N points will determine a unique curve of degree at most (N-1).
The graph of an equation is a visual representation of the values that satisfy the equation.

The graph of a quadratic equation is a parabola
It depends on the graph. There may or may not be an equation.
A non-linear graph. It could be a polynomial (of a degree greater than 1), a power function, a logarithmic or trigonometric graph. In fact any mathematical function other than a linear equation.
There are infinitely many options. The equation could be a polynomial of degree greater than 1, or it could be a power function, a log function or any combination of these with trig functions. The problem is exacerbated by the fact that there is no clue in the question as to what a stands for.
You find the equation of a graph by finding an equation with a graph.

A parabola is a graph of a 2nd degree polynomial function. Two graph a parabola, you must factor the polynomial equation and solve for the roots and the vertex. If factoring doesn't work, use the quadratic equation.

You need to look at the graph first. The shape of the graph will give you hint as to what general form of equation would produce such a graph, then based on the numerical values for the dependent variable, you shouuld be able to figure out the exact equation or family of equations for the graph. For example what equation would produce a straight line, a curve--parabola, a circle, an eclipse, etc.? For your referance, you may review the (Conic Sections of Algebra or Precalculus). However, in higher level mathematics like Calculus, there are specific techniques in doing that but it's for me to discuss. Only well skilled mathematicians would discuss that for you. If you will, you may copy and past the particular graph you are referring to, together with the question exactly as it comes from your source.
