Fossil fuel in itself is simply an example of potential energy, which can be changed to mechanical energy by burning, thus changing from potential energy to heat energy and then to mechanical energy.
As far as I know, there is no such thing as "mechanical heat energy"; there is mechanical energy, and there is heat energy.
That is normal for example when energy is transformed from mechanical directly into electrical i a generator, or electrical to mechanical in a motor.
Mechanical energy is heat light and kinetic energy but instead of saying something has heat light and kinetic energy we just say mechanical! :)
Check out the fundamental limits of turning Mechanical Energy to Heat energy.
Smoke is an example of mechanical energy and heat energy. This is because the internal motion of atoms is called heat energy and mechanical energy is energy associated with motion. Smoke contains atoms moving which is both an example of heat and mechanical energy.

Absolutely a simple example is the engine.

It will still be heat energy, but it can be converted to mechanical energy
Friction, for example the heating in brake discs and pads when slowing a vehicle

It isn't. Chemical energy is converted into mechanical energy.
A heat engine is a system that converts heat to usable energy, particularly mechanical energy, which can then be used to do mechanical work
