Generally, when there is a warrant for your arrest, when the officer has seen you commit an arrestable offense, or when there are witnesses or other evidence that you have committed an arrestable offense.

If the body of the offense report contains (or withholds) material information that alters the offense reporte this is a serious and indictable offense. The officer stands a good chance of losing their job AND being charged criminally.
A police officer would write a supplemental report to add evidence or clarify information he may have received after the arrest.
Complain to his superiors, or the Internal Affairs Division of his department (if it has one) - if it results in a criminal prosecution then tell the prosecutor. If all else fails contact your local newspaper. BUT!!! You'd better have good proof of what you are saying or you could be charged with the criminal offense of making a "False Report to the Police," or the officer could sue you for 'defamation.'

Obstructing a police officer is a serious offense. If the obstuction occurs as the officer is investigating, you could be charged with a felony in court. In other situations, it would likely be a misdemeaner.
