Depending on which tooth is involved, either might be more appropriate.

If the permanent tooth that is going to replace the baby tooth is not expected to erupt (come into the mouth) for several more months or years, a pulpotomy is probably indicated. If a baby tooth is lost too early, it can lead to significant problems with the alignment of the permanent teeth later on.

If a pulpotomy (root canal on a baby tooth) is not done, for whatever reason, the dentist may need to place a space maintainer to hold the position open. This can also lead to more tooth decay, because the space maintainer may be difficult to keep clean.

There is a fallacy out there that says it isn't cost effective to preserve primary teeth because they will be lost eventually anyway. Nothing could be further from the truth. Besides allowing a child to chew and properly digest their food, and display an attractive and healthy smile, baby teeth are critical in the proper development of the jaws and facial bones. Premature loss of baby teeth can lead to costly orthodontic problems with the permanent teeth. Far more than a cosmetic concern, it can result in improper occlusion (the way teeth come together) and this can ultimately lead to severe dental decay and even TMJ (jaw joint) problems.
