One of things it means is saying that the Holy Spirit is the devil. This is a hardness of the heart which cannot be forgiven for the person has denied who God is and attributed him with evil. Therefore the person will never seek repentence for they do not know God or accept him.

Almost no one. This requires an almost certain knowledge of God and Jesus Christ and then to turn away, putting him to open shame, and crucifying him anew. Not too many people are qualified.

There is a lame movement where people record themselves saying that they deny the Holy Ghost, but they have never had it, so are ineligible to deny him.

We are told not to blaspheme (Colossians 3:8) but if we do, like the Apostle Paul did (see 1 Timothy 1:12-13) we still can be forgiven via repentance. That being said, the question seems to be about the 'unpardonable sin' or blasphemy against the Holy Spirit.

The unforgiveable sin is found in Mark 3:28-29. This has to do with denying the authority and power of God. Mark, in verse 30, explains that Christ was responding to an accusation from His detractors, who said, "He has an unclean spirit". They meant that His authority came from Satan, not from God. Although these accusers knew that Christ was the Son of God, they accused Him of being a fraud. Christ warned them that they were in danger of committing a sin for which there was no forgiveness.

Of course God is willing to forgive all of any sin since one is not lesser or greater that the other. Like the Apostle Paul, the sinner must repent - heartfelt repentance or face the 2nd death in the Lake of Fire at the Judgment Process (Matthew 10:28; Revelation 20:14-15).

It is believed by some evangelicals, that if one accepts the call of the 'first fruits' and later turns back willfully to their old sinful ways, in effect discounting the call of Salvation by God's Grace, these 'few' will not be forgiven as there is no other covering for their sins - these had been led to Christ by the Father and their sins have been covered by the death and resurrection of Jesus which they now discount (see Hebrews 10:26).

That's a good question, and different people have different interpretations. The phrase occurs in The Bible in the following context: Jesus had been "casting out spirits," and some of the Pharisees accused Jesus of doing so by the power of "Beelzebul, Prince of Demons." Shortly after this Jesus says that all sin and blasphemy against the Son (Jesus) will be forgiven, but that blasphemy against the Holy Spirit shall not be forgiven. Many feel that the comment is directly related to the accusation, and that "blasphemy against the Holy Spirit" is what the Pharisees were doing when they attributed something done by the power of the Holy Spirit to something done by the devil. Others suggest that it amounts to acknowledging that God is sovereign and that He offers forgiveness, and rejecting that forgiveness anyway. The main argument for this seems to be that in every other case where forgiveness is mentioned it's suggested that anyone who asks forgiveness will be forgiven. In this interpretation, it's not so much that the sin itself is inherently unpardonable as that anyone who commits it will not ask forgiveness, since it's already been rejected.
