The term conclusion means an ending. Therefore, a concluding sentence is part of the last paragraph. However, every paragraph should end smoothly and lead easily into the next paragraph.
(I'm answering my own question >_>) "In a nutshell" is one of my favorite concluding phrases. Others are "In conclusion," "All in all," or just simply state your sentence.
