Disease Management is dealing with the reality of a disease and preparing for it. Disease management is simply managing what you must do to keep your disease in check. Taking your medications daily and on time is disease management. Checking your blood pressure or blood sugar is disease management. Diabetics need to check the soles of their feet as part of their disease management.
