If you want it in present simple or future tense it would be 失敗します /shi-ppai shi ma su/ and if it's intended to mean in the past tense 失敗しました /shi-ppai shi ma shi ta/.

For failing in an exam 落ちます /o chi ma su/ in present/future tense and 落ちました /o chi ma shi ta/ for past tense can also be used.
